Our community-based specialist palliative care team provides symptom assessment, guidance, and support to patients, families, and GPs, helping you stay as comfortable and as well as possible at home.
Who is this service for?
Our community palliative care service is for patients living with a life-limiting illness who are being cared for at home. The team works alongside your GP, district nurse, and hospital team to provide an additional layer of specialist support.
Whether you need help managing symptoms, want someone to talk through what's ahead, or simply need reassurance that support is there, our team is here.

Your journey with us
From referral to ongoing support, here's what to expect at each stage.
Welcome
Once we receive and accept your referral, a member of our Community Triage team will contact you within two working days.
Getting in touch
We offer various methods of contact including telephone, virtual appointments, Hospice outpatient clinics, or where appropriate, a visit at home.
Assessment
You'll receive a regular assessment, daily, weekly, or fortnightly depending on your situation and symptoms.
Working with your GP
Following your assessment, we continue to update your GP and other professionals involved in your care to ensure everything is joined up.
Ongoing support
Our team is available 24/7 for you, your family, and your primary care team. We're just a call away, 0141 952 1141.
Discharge and re-referral
If you or we feel you no longer need the service, we'll contact your GP. You can return at any time you or your GP feel it would be helpful.
Important to know
Our team provides symptom assessment, guidance, advice and support to you, your family, and your GP. It is only your GP who can prescribe medication, and only a pharmacy can dispense medication. We work closely with both to ensure you get what you need.
