Social Work Service Provision
Provide social work services as underpinned by SMoSH policies, best practice statements, current research, benchmarks and initiatives.
· Develop, maintain and review internal systems of referral to Social Work services via audit
· Develop information leaflets for Social Work Services within the Hospice, outlining the role of social work within palliative care.
· Undertake an assessment of patient/family/carer needs, develop and review the plan of care, providing feedback using the departmental meeting mechanism
· Ensure all patients admitted to the Specialist Palliative Care Centre - SJW are aware of the Hospice Social Work Service by introduction.
· Attend and contribute to weekly multidisciplinary team meeting and the Day Hospice/Community Specialist Palliative Care Meeting updating team members accordingly with case progress or completion.
· Attend and contribute to case conferences and complex discharge planning opportunities supporting effective collaboration of both Hospice and Primary Care Teams alongside Community Social Work.
· Provide patients and families/carers with advice on available benefits and grants, supporting and guiding the application process where needed..
· Undertake regular review and audit of the social work service against recognised Social Work guidelines and regulations, Health Improvement Scotland and Social Care and Social Work Improvement Scotland standards and recommendations.
